Specialized Care for Seniors
Senior Services provides several areas of specialized care including palliative care, geriatric care, long-term care, pre-operative assessment and specialist consultations.
Palliative Care Assessment
For patients who have complex, serious, life-threatening or debilitating chronic diseases, Senior Services will arrange for an assessment with a physician specializing in palliative care. Palliative care provides and interdisciplinary approach to optimize quality of life and address pain and symptom management, advance care planning, transition of care, ethical issues or concerns, and family/patient communication.
Geriatric Assessment
Patients receive a comprehensive, geriatric assessment including a complete health and social history, and physician examination by a geriatrician, a physician specializing in the care of older adults and the frail elderly. The program focuses on optimizing each patient's quality of life and functional ability.
Long-term Care Physician Group
Physicians dedicated to caring for the frail elderly and seniors in skilled nursing facilities provide on-site physician services, medical direction (through our medical directorship program) and can facilitate access to hospital services and consulting specialists as needed.
Pre-operative Assessment
Senior Services can coordinate a pre-operative physical assessment and work-up prior to a patient's scheduled surgical procedure.
